The Rural Nurse Resource Course “Foundations of Faith Community Nursing Practice”, was approved for 36.0 contact hours under the following CNE activity ID#: 13515-04-12.

Registration / Application
Course fee is $395.00. Make checks payable to Texas Health Presbyterian Hospital Plano.
We have scholarships for THP RN’s and Covenant Congregation RN’s.
You will be required to purchase 2 textbooks ($45.00) on the first day of the course.
To receive contact hours and certificate of attendance you must sign-in each day, attend the entire course, participate in course activities and return a completed evaluation form.
Registration deadline is July 10, 2012

A letter stating your personal goals for attending the program, and a letter of support from your clergy/minister must accompany the registration form. Please mail your application, purpose and goals letter and letter from clergy /institution to:
